Abstract

ABSTRAK Latar Belakang: Leptospirosis merupakan penyakit zoonosis yang masih menjadi masalah kesehatan di Indonesia. Provinsi Daerah Istimewa Yogyakarta melaporkan 168 kasus dengan 13 kematian (CFR 7,74%). Berdasarkan data surveilans Dinas Kesehatan Kabupaten Bantul tahun 2023–2025, tercatat 337 kasus leptospirosis yang tersebar di seluruh kapanewon, dengan Kapanewon Bantul mencatat 52 kasus pada tahun 2025 sebagai salah satu wilayah dengan jumlah kasus tertinggi. Tingginya kejadian leptospirosis meningkatkan risiko paparan bakteri Leptospira, terutama pada aktivitas yang berkontak dengan air atau tanah tercemar, sehingga penggunaan alat pelindung diri (APD) menjadi upaya penting dalam pencegahan penularan. Metode: Penelitian ini menggunakan desain cross sectional dengan sampel 80 responden yang ditentukan melalui rumus Slovin dan dipilih menggunakan teknik proportional stratified random sampling. Data dikumpulkan melalui lembar observasi dan kuesioner, serta dianalisis menggunakan uji Chi-Square. Hasil: Tidak terdapat hubungan antara usia (p=0,444), jenis kelamin (p=0,523), tingkat pendidikan (p=0,590), pekerjaan (p=0,365), dan lingkungan kerja (p=0,137) dengan kepatuhan penggunaan APD. Terdapat hubungan antara riwayat luka (p=0,000), kesesuaian penggunaan APD (p=0,001), kendala penggunaan APD (p=0,017), tanda-tanda keberadaan tikus (p=0,007), dan genangan air (p=0,014) dengan kepatuhan penggunaan APD. Jenis APD yang paling banyak digunakan adalah pakaian lengan panjang dan alas kaki. Kesimpulan: Variabel usia, jenis kelamin, tingkat pendidikan, pekerjaan, dan lingkungan kerja tidak menunjukkan hubungan yang signifikan dengan kepatuhan penggunaan APD. Sebaliknya, riwayat luka, kesesuaian penggunaan APD, kendala penggunaan APD, tanda keberadaan tikus, dan genangan air merupakan faktor yang secara signifikan berhubungan dengan kepatuhan penggunaan APD pada penderita leptospirosis di Kabupaten Bantul. The Special Region of Yogyakarta reported 168 cases with 13 deaths (CFR 7.74%). Based on surveillance data from the Bantul Regency Health Office for 2023–2025, there were 337 cases of leptospirosis spread across all sub-districts, with Bantul Sub-district recording 52 cases in 2025 as one of the areas with the highest number of cases. The high incidence of leptospirosis increases the risk of exposure to Leptospira bacteria, particularly during activities involving contact with contaminated water or soil therefore, the use of personal protective equipment (PPE) is a critical measure in preventing transmission. Objective: This study aims to determine the relationship between host, behavioral, and environmental factors and PPE compliance among leptospirosis patients in Bantul Regency. Methods: This study employed a cross-sectional design with a sample of 80 respondents determined using the Slovin formula and selected via proportional stratified random sampling. Data were collected through observation forms and questionnaires and analyzed using the chi-square test. Results: There was no association between age (p=0.444), gender (p=0.523), educational level (p=0.590), occupation (p=0.365), and work environment (p=0.137) and PPE compliance. There was an association between history of injuries (p=0.000), appropriateness of PPE use (p=0.001), barriers to PPE use (p=0.017), signs of rodent presence (p=0.007), and standing water (p=0.014) and compliance with PPE use. The most commonly used types of PPE were long-sleeved clothing and footwear. Conclusion: The variables of age, gender, educational level, occupation, and work environment did not show a significant association with PPE compliance. Conversely, history of injury, appropriateness of PPE use, barriers to PPE use, signs of rat presence, and standing water were factors significantly associated with PPE compliance among leptospirosis patients in Bantul Regency.
